Job Summary:
The Case Manager at Silence Aloud Inc.s The Bridge Wellness Center. Homeless Youth Drop-In Center will provide trauma-informed, strengths-based, and youth-centered case management services to young people experiencing or at risk of homelessness. Case Managers will work closely with youth (ages 16-24) to assess needs, develop individualized service plans, connect them to critical resources, and support them in achieving long-term stability. The role requires a deep understanding of the barriers faced by unhoused youth, including justice-involved youth, LGBTQ+ youth, survivors of trafficking, and those with disabilities or mental health challenges.
Key Responsibilities:
Direct Client Support & Case Management:
Conduct comprehensive intake assessments to identify needs related to housing, employment, education, health, and well-being.
Develop individualized service plans with youth, setting measurable goals and action steps for stability and self-sufficiency.
Provide intensive case management, including crisis intervention, safety planning, and emotional support.
Monitor youth progress, adjusting case plans as needed to reflect evolving goals and circumstances.
Assist youth in securing vital documents such as birth certificates, IDs, and Social Security cards.
Housing & System Navigation:
Assist youth in navigating housing programs, including rapid rehousing, transitional housing, and permanent supportive housing options.
Serve as a liaison between youth and Riverside Countys Coordinated Entry System (CES) & HomeConnect, ensuring access to housing resources.
Provide education on tenant rights, lease agreements, and independent living skills.
Resource Connection & Advocacy:
Link youth to community resources, including mental health and medical services, substance use treatment, legal aid, education and employment support, childcare, and life skills training.
Advocate for clients within public assistance programs, including Medi-Cal, CalFresh, and General Relief.
Assist with referrals to employment services, vocational training, and financial literacy programs.
Collaboration & Community Engagement:
Work closely with peer navigators, mental health clinicians, legal aid providers, and other interdisciplinary team members to ensure holistic support for each youth.
Develop and maintain strong relationships with housing providers, social services, schools, juvenile justice agencies, and local employers.
Participate in case conferences, program meetings, and collaborative community efforts to improve service coordination.
Documentation & Compliance:
Maintain detailed, accurate, and confidential client records in alignment with organizational and funding requirements.
Ensure timely documentation in the Homeless Management Information System (HMIS) and other data tracking platforms.
Collect and report data on program outcomes and success stories to inform continuous improvement efforts.
Qualifications & Experience:
Bachelors degree in Social Work, Psychology, Counseling, Human Services, or a related field (Masters preferred).
Minimum of two (2) years of experience in case management homeless services, youth services, or social work.
Experience working with youth facing homelessness, justice system involvement, trauma, sexual exploitation, substance use, and mental health challenges.
Knowledge of trauma-informed care, harm reduction, positive youth development, and crisis intervention strategies.
Strong understanding of housing and homeless services systems, including CES, HUD-funded programs, and youth-specific resources.
Bilingual (English/Spanish) preferred.
Additional Requirements:
Must pass background check and TB test.
Valid California drivers license and reliable transportation required
Flexibility to work some evenings and weekends as needed.
