HIV Health Navigator (Part-Time, 0.2 FTE)
Esperanza Health Center – Philadelphia, PA
Schedule: 8 hours/week (Tuesdays & Thursdays, flexible hours)

Position Summary
We are seeking a compassionate and motivated HIV Health Navigator to join our team. The Navigator will work closely with patients living with HIV to support their engagement in care, encourage adherence to treatment, and help prevent complications through regular outreach and connection to resources. Special emphasis will be placed on supporting African American patients and others experiencing disparities in HIV care and outcomes. This role includes identifying and addressing barriers related to Social Drivers of Health (SDOH) and promoting holistic wellness.

Key ResponsibilitiesCare Engagement & Outreach
Contact patients who have missed appointments or are overdue for lab work to encourage re-engagement in care.
Track and re-engage patients who are lost to follow-up (90+ days without HIV services).
Maintain up-to-date patient records, including preferred methods of contact.
Attend community events (e.g., health/resource fairs) and foster strong relationships with partner organizations to support patient outreach.
Resource Navigation
Identify barriers to viral load suppression, including SDOH such as housing, transportation, mental health, and substance use.
Connect patients to EHC and community resources, including HIV Peer Support Groups, Behavioral Health, Lifestyle Medicine classes, and EHC’s CORE Fitness program.
Collaboration & Documentation
Partner with the care team to develop individualized outreach plans.
Document all patient encounters and outreach activities in alignment with HIV Department protocols.
Attend departmental meetings and EHC Care Team Meetings twice a month.
Spiritual Care
Offer spiritual encouragement and prayer to patients who are open to receiving it.
Participate in personal and team spiritual growth through staff devotions and retreats.
Communicate the love of Christ through compassionate, respectful, and hopeful interactions.
High School Diploma or equivalent required.
