Job Description
We are seeking a compassionate and dedicated Patient Navigator/Case Manager to join our team as part of the HRSA-funded Quality Treatment Initiative-Transforming Justice Initiative (QTI-TJI) grant. This unique role focuses on addressing health disparities and improving health outcomes for individuals transitioning from the justice system. The Patient Navigator will work directly with jail inmates, providing essential case management services, and also maintain an external office for follow-up care coordination and patient support in the community.
Key Responsibilities:
Direct Support Within the Jail Facility:
Build trusting relationships with incarcerated individuals to understand their health needs, facilitate access to medical care, and assist with discharge planning.
Case Management:
Develop personalized care plans for each patient, connecting them with resources for ongoing healthcare, substance use treatment, mental health services, housing, and other community supports.
Care Coordination:
Ensure a smooth transition from incarceration to the community by coordinating follow-up care, facilitating access to healthcare services, and assisting patients in navigating the healthcare system.
Empowerment and Independence:
Foster patient independence by helping individuals learn how to navigate healthcare systems, access services, and manage their own health effectively.
Resource Connection:
Assist patients in connecting with social services, housing programs, legal support, and other community-based resources to address barriers to care and improve overall outcomes.
Community Reintegration:
Support patients as they reintegrate into society, ensuring that they have the necessary tools and support to thrive in the community after release.
